Lecture Description: Advances continue in the genetics and other putative causes of ALS/MND at a remarkable rate, including increasingly more targeted treatment trials. Despite the remarkable progress in these areas, the core supportive management and use of palliative care remains relatively unchanged. This talk will focus on the basic principles in supportive management starting with the “breaking the news” session to integrating the ALS/MND patient into a multi-disciplinary program where they find they can feel physically and psychologically comfortable. Palliative Care is an important component of management and essentially begins when the diagnosis is made.

Prof. Paul Barkhaus

Dr. Barkhaus is a Professor of Neurology and Physical Medicine & Rehabilitation and founder and Director of the ALS Program as well as, Director of the Neuromuscular Diseases Program at the Medical College of Wisconsin. Dr. Barkhaus completed his medical education and training at Wayne State University in Detroit followed by a fellowship in EMG at the University of Minnesota under Dr. William Kennedy and additional training in quantitative EMG in Uppsala Sweden with Dr. Eric Stålberg and Duke University with Dr. Donald Sanders. Also, Dr. Barkhaus began a productive collaboration in EMG research with Dr. Sanjeev Nandedkar, who at the time, was a research engineer in the Duke EMG laboratory and currently is a senior consultant here at Natus Medical. Drs. Barkhaus and Nandedkar have been collaborators since 1986 on multiple research and educational projects. Additionally, Dr. Barkhaus has authored or coauthored 65 peer-reviewed publications and 40 peer-reviewed books, chapters, and reviews. He received the Distinguished Researcher Award from the American Association of Neuromuscular and Electrodiagnostic medicine in 2015.

Course introduction:
The American College of Surgeons Trauma Quality Improvement Program recommends that therapy for severe traumatic brain injury be individualized and directed by multimodal neuromonitoring data. Implementing a successful multimodal neuromonitoring program requires the developing of a high-functioning ICU care team composed of neurosurgeons, neurointensivists, neuroscience nurses, and respiratory care professionals. This course, presented by Lawrence Chyall RN, MS, CNS from San Francisco General Hospital, will use case studies to demonstrate how the TBI team can work together to gather, interpret, and use multimodal neuromonitoring data to improve care.
Learning Objectives:
At the conclusion of this seminar the participants will be able to:
- Explain how cerebral autoregulatory status might change the approach to support cerebral perfusion
- Demonstrate how cerebral compliance can be quantified using the pressure equalization ratio and how this impacts care
- Describe the role of cortical surface electrodes in multimodal neuromonitoring
- Understand the role of the bedside RN, EEG Technologist and RCP in therapy directed by neuromonitoring data

Lawrence Chyall is the Neuroscience Clinical Nurse Specialist and Neuroscience Programs manager at Zuckerberg San Francisco General Hospital. His research and clinical interests focus on continuous EEG and ECOG in patients with severe TBI and ICU and on the use of transcranial magnetic stimulation in patients with spinal cord injury. He is committed to expanding the role of neuro ICU nurses to integrate EEG into their practice. Lawrence holds degrees from UC Berkeley and UC San Francisco, both of which he attended as an adult learner, coming to nursing as second career following a 15 years as a sommelier. Failing to pass the Theory portion of the Master Sommelier examination he traded his tastevin for a stethoscope without regret.

She originally is a biomedical engineer with a Master’s degree in Biotechnology/Microsystems from Grenoble Institute of Technology (France). Dr. Lawrence Hirsch, Professor of Neurology at Yale University, will discuss the use of real-time or close to real-time neurotelemetry; methods of obtaining rapid initial EEGs; detecting events other than seizures using quantitative EEG (including ischemia, high intracranial pressure, etc); and invasive EEG recordings. This will be done via review of the literature, especially recent publications, and sample cases.
Learning Objectives:
At the conclusion of this seminar the participants will be able to:
- Recognize the importance of rapid diagnosis of nonconvulsive status epilepticus.
- Be aware of emerging devices for obtaining rapid initial EEGs.
- Know how to identify ischemia, changes in intracranial pressure, and other acute changes in brain function via quantitative EEG.
- Recognize the potential clinical utility of intracranial EEG recordings in the critically ill.

Lawrence J. Hirsch, M.D. is Professor of Neurology, Chief of the Division of Epilepsy and EEG, and Co-Director of the Comprehensive Epilepsy Center, all at Yale University. He has held leadership positions in the American Clinical Neurophysiology Society (ACNS), American Epilepsy Society, American Academy of Neurology, and the Epilepsy Foundation, and is lead author of the 2012 and 2021 ACNS guidelines on critical care EEG terminology. He is founder and former chair of the Critical Care EEG Monitoring Research Consortium, which now includes more than 50 centers, and co-chair of the medical advisory board of the NORSE Institute. He has been an active researcher throughout his career, having published more than 200 peer-reviewed manuscripts and more than 100 reviews, editorials or book chapters on topics including status epilepticus, all aspects of EEG including intracranial recordings and brain monitoring in the critically ill, brain stimulation, epilepsy surgery, seizure clusters, rescue medications, sudden death in epilepsy (SUDEP), and anti-seizure medication use. He has won multiple teaching awards and is co-author of the first-ever atlas on EEG in critical care.

This course reviews the basis of neuronal oscillations and their context in diagnosis and treatment of epilepsy patients and recent findings on the role of neuronal oscillations in the near-death phase and end-of-life cycle.

Dr. Ajmal Zemmar

Dr. Ajmal Zemmar is a neurosurgeon at the University of Louisville Health in Louisville, KY. Dr. Zemmar obtained his medical degree from the Goethe University-Frankfurt am Main (Germany) and a PhD in Neuroscience at the Brain Research Institute Zurich, jointly from the Swiss Federal Institute of Technology (ETH) and the University of Zurich (Switzerland). Dr. Zemmar is also a physician-scientist specializing in epilepsy and functional neurosurgery. His research interests are the scientific basis of death and near-death experiences as well as neuro-robotics. When he was a young child, Dr. Zemmar and his family fled the war during the Russian invasion in Afghanistan and immigrated to Germany. His medical training has taken him around the globe from Germany to Switzerland, the United States, England, Canada, China and South Africa. He was trained at prestigious institutions including Oxford University, New York University, the Max-Planck Institute for Brain Research and Toronto University learning from and working alongside some of the most acknowledged neurosurgeons and neuroscientists in the world. Dr. Zemmar speaks Farsi, German, and English fluently in addition to knowing Spanish and Latin. Dr. Zemmar holds membership in multiple international neurosurgical and medical societies, has been the recipient of numerous awards, and serves as an ad-hoc reviewer for many internationally recognized medical journals. During his spare time, he enjoys spending time with his wife and daughter.
